The Steelers were out of challenges, so Cleveland kept possession.
With the game on the line late in the fourth quarter, Trent Richardson coughed up a fumble that could have resulted in a Steelers comeback to end the day. However, the refs ruled Richardson down, and Pittsburgh couldn't do anything about it.
Trent Richardson pretty clearly just fumbled, refs ruled him down, Steelers out of timeouts with which to challenge.

As you can see here, it looks like the ball comes loose. I'm not an official referee, but I probably could have made this call.

It just wasn't Pittsburgh's day. Charlie Batch threw two interceptions, including one the bounced around and ended up in the hands of a defensive tackle. To add on to the misery, Rashard Mendenhall, Isaac Redman, Jonathan Dwyer and Chris Rainey all lost fumbles on the afternoon.
Anything and everything that could have gone wrong seemed to for the Steelers in Week 12.
